Thank you for choosing our practice to serve your dental needs. Below are the new patient registration forms that you may print out and complete in the comfort of your own home. Please be sure to bring these completed forms with you for your initial consultation. As a new patient you will find the following information very helpful.
Your First Appointment
Please print a copy of the New Patient Forms prior to your first visit.
Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. An involved medical history or treatment plan will require an evaluation first, and then a second appointment to provide treatment on another day.
Please assist us by providing the following information at the time of your consultation:
- A panoramic x-ray, if applicable (no older than 1 year).
- A list of any medications you are currently taking.
- If you have dental and medical insurance please bring your cards.
- We request that you complete our new patient forms and bring with you.
- A referral from your general dentist.
Important: All patients under the age of 18 years of age must be accompanied by a parent or guardian at the consultation visit.
Pre-Op Instructions (IV Anesthesia)
- Do not eat or drink for six hours prior to your scheduled appointment. This includes water and chewing gum.
- Bring someone to stay at the office during surgery and drive you home. Someone must stay with you the rest of the day.
- Arrive 10 minutes prior to appointment time for check-in.
- Late arrivals, 15 minutes after appointment time, may be asked to reschedule.
- Wear clothes that are loose-sleeved, comfortable, and cool.
- Minors must be accompanied by a parent.
- Brush your teeth before your appointment, but do not swallow water.

Post-Op Instructions
Bleeding and oozing are to be expected and may persist for several hours. You may control it with gauze pressure as necessary. Ice can be applied externally to your face to help limit the swelling. Use ice for 30 minutes on and 30 minutes off for 48 hours. Pain is expected, therefore you may take any over-the-counter medication you routinely use for a headache, or if pain medication has been prescribed you should take as directed. Soft foods are appropriate for the first few days after surgery. Keep your mouth as clean as possible with GENTLE rinsing and brushing. If you smoke, it is best not to do so for several days following oral surgery to allow for uneventful healing. If a Monoject syringe was given to you, begin using it as directed 3-4 days following surgery.
